Here is what you should do next to complete the application:
- Complete the questionnaire below. The questionnaire will require documentation. You may need help finding and uploading the documentation.
- After you complete the first step, we will schedule an interview in June. This will be a conversation with the staff about the program. You can also ask questions.
This questionnaire requires documentation. There is an upload at the end of this form.
- We recommend preparing the required documents before completing the questionnaire.
- If you need to stop filling out the questionnaire, it’s okay. There is an option to Save as a Draft, then continue filling out the form later.
The following documentation is required for this step of the CCS application. You will be asked to upload these documents at the end of this form. You may want to get the documents ready before starting the questionnaire. However, you will have an option to complete and submit all other information, then contact us to complete the documentation section at a later time.

- High school transcript
- Two letters of recommendation. These can be from anyone who knows you and can explain why they believe this program would be a good fit for you. Potential sources: teachers, principal, IEP team members, employment specialist, family or community contact.
